A unicyclic, aminoketone antidepressant. The mechanism of its therapeutic actions is not well understood, but it does appear to block dopamine uptake. The hydrochloride is available as an aid to smoking cessation treatment.

 Pharmacologic Action
  • Second Generation Antidepressive Agent
    A structurally and mechanistically diverse group of drugs that are not tricyclics or monoamine oxidase inhibitors. The most clinically important appear to act selectively on serotonergic systems, especially by inhibiting serotonin reuptake.
  • Dopamine Uptake Inhibitor
    Drugs that block the transport of Dopamine into axon terminals or into storage vesicles within terminals. Most of the adrenergic uptake inhibitors also inhibit dopamine uptake.

Bupropion (Zyban, Wellbutrin SR): reports of deaths, seizures, serum sickness
In a letter to health professionals,1 Health Canada and GlaxoSmithKline stated that 1127 reports of suspected adverse drug reactions for bupropion were received between May 1998 and May 28, 2001. Included were reports of 19 deaths (including 1 case of liver failure, 1 case of myocarditis, 3 cases of suicide), 172 reports of seizures or convulsions and 37 reports of serum-sickness-like reactions. Although cause- and- effect relations have not been established in the vast majority of the reports, it is important to remind physicians and warn patients of possible adverse effects of this medication.

When the anti-depressant bupropion was introduced in the United States in 1989, the specific pharmacologic basis of its clinical effects was uncertain. Research conducted over the past decade has significantly advanced the understanding of the neuropharmacology of bupropion and has demonstrated a novel mechanism of antidepressant activity. This article discusses the mechanism of action of bupropion and relates the drug's neuropharmacologic effects to its clinical efficacy and tolerability profiles.
