The Babinski sign legacy

It thus appears that the Babinski sign is an indication of withdrawal of supraspinal control of flexor reflexes in the lower limbs. Clinically it can be equated with inactivation, transient or permanent, of the upper motor neurone, which term implicates corticoreticulospinal fibres as well as the pyramidal tract anywhere in its path from cerebral cortex to termination in the cord.
Flexor reflexes are prominent in the newborn and young infant. In order for the baby to stand, flexor reflexes in the lower limbs must be brought under control by the dorsal reticulospinal tract. For the baby to walk the flexor synergy must then be harnessed to the motor cortex as part of the walking pattern. With the maturation of upper motor neurone pathways the Babinski sign disappears and the great toe goes down on stimulation of the sole. If upper motor neurone control is temporarily suspended, as in an epileptic fit, or is abolished by disease the Babinski sign reappears.

In 1896, Joseph Babinski, a French neurologist, first described the best known neurologic eponym 'the Babinski sign'. This sign is characterised by dorsiflexion of the big toe and recruitment of the extensor hallucis longus muscle, on stimulating the sole of the foot. He has emphasised from the outset, the intimate relationship between this sign and the shortening movement in other leg muscles, which form the flexion synergy of the lower limb. The Babinski sign is not a new reflex, rather it is released as a result of breakdown of the harmonious integration of the flexion and extension components of the normal defence reflex mechanism, due to pyramidal tract dysfunction. A pathological Babinski sign should be clearly distinguished from upgoing toes that may not always be a part of the flexion synergy. This article reviews the Babinski sign in detail, focusing on the historical perspectives, role of pyramidal tract dysfunction and art of elicitation and interpretation. The significance of assessing this phenomenon in the entire leg, and the clinical clues that will help to dispel the myths regarding the Babinski sign, have been emphasised.

Before Joseph Babinski's discovery in 1896, knowledge about reflexes was limited. Although reflex activity was recognised as an involuntary response of the nervous system by Descartes as early as 1662, it was not until the 19th century that clinicians began to understand the underlying mechanisms. Motor and sensory roots were described in 1811, followed by the neurone theory in the latter half of the century. Cutaneous and tendon reflexes were defined in rapid succession from 1875. Flexion reflex withdrawal of the lower limb was recognised as indicative of a pyramidal syndrome, but little attention was given to the movement of the toes.
